Have only just skimmed so far but looks like the hyptothesis is that with physical signals of agony, the lungs can synthesize large amounts of DMT.

They used the word "large", though that is probably relative to normal endogenous production in the body.

They also talk about the non-hallucenogenic, neuroprotective properties and actually end the paper by suggesting that DMT could be useful in emergency care involving cardiac arrest, stroke, etc. Talk about having a story to tell after surviving a heart attack, producing your own endogenous DMT in your own body, then having moar injected to make sure they save your life at the hospital!
