I'm no expert, but it pretty much sounds like you should either quit using LSD or find a new source. Your reaction sounds rather extreme to me, but not unheard of. I am no expert on vasoconstriction but I do know it can be very uncomfortable to downright scary.
Which brings me to my next point. If you are not in tip top health, DMT may not be the drug for you. It has a fairly heavy body load that users tend to amplify with what is sometimes called pre-launch anxiety. It can and will raise both heartrate and blood pressure.
I would say that DMT at decent doses does have a reputation for inducing ego-deaths, but mainly I would say that you should try to go in with all assumptions on hold. In my experience, DMT tends to give me what I need rather than what I think I want.
Even small amounts of DMT, correctly vaped in a short amount of time can radically distort reality and induce a heavy tripping state reminiscent (to me) of a very very heavy LSD dose. But, DMT at even midlevel doses tends to take me deeper. It induces a trance and believable, immersive, 3D gorgeous geometric visions. Sometimes at higher doses it is indescribably beautiful and so separates me from being in true physical touch with my body that I am convinced I am dead. Basically it feels like it blows or launches my spirit right out of my body.
I hope this helps, but I know what will help most is just continuing to read and research.
Also, I cannot recommend extracting your own DMT enough. It removes so many concerns and questions and gives you a decent amount to work with. You can find extraction teks for various plant based entheogens in the wiki link above.
Okay, here's hoping your journey has just begun. Again, welcome to the Nexus.
