For verification of legality, i would recommend consulting with a practicing lawyer in order to eliminate any necessary legal loopholes that would need to be addressed. In addition, I would urge the need to get your document notarized once it's prepared--and laminate it!

Talking to a lawyer seems like a good idea... but notarizing it? Isn't that incriminating myself and the person taking the entheogen (in the case of most entheogens)? Would witnesses help? These are excellent questions for the lawyer when he is consulted. I would have to say that this step cannot be overlooked if we really want to make our existence official with the assurance of as little legal trouble as possible. I wish i had the answers to share, but I'm no lawyer.
