When is grounding enough versus when do they need to seek real help? Options

In general most of the people I know into psychedelics are into the large doses a few times a year, a lot of the times this leaves people reeling. So far I feel like everyone has been very good about grounding and we haven't had any breaks. However, the time to ask these questions isn't after I am concerned about a friend.

My default answer when someone is having a really hard time grounding, I will usually let them just talk to me for a while, sometimes getting it all out will help sort it all out.

The next suggestion is to Sleep 9 hours 4 days in a row, Eat fruits and vegetables and protein, walk 2 miles a day with music or audio book that keeps your mind off of the experience, hanging out with friends who you wouldn't talk about this stuff with, taking off your shoes putting your feet on the ground saying your name, where you are physically in the world and what your next 2 tasks will be (don't do this until you are ready for your tasks. Meditation or prayer and staying away from anything they believe could move them further in the wrong direction.

At what point does the above become worthless and you need to suggest psychiatric care? I know there is no "hard and fast rule" but any experiences where you had to tell a buddy to get real help?
