Bufotenine is found in urine of all people it is not only produced in the CNS but in the periphery as well. It's levels in the cerebral spinal fluid, urine and other tissues have not been found to vary that much between patients.

Although there are many many possibilities however it is likely a pharmacokinetic difference although pharmacodynamic (receptor subtypes, levels of receptor expression) may also be involved.
